All student-athletes wanting to participate in interscholastic athletics during any season, must have the following paperwork completed before the first day of practice/tryouts.
1. Online Registration: Each season student-athlete and parent must register for the sport that the student will be participating. This will complete the Pre-Participation Physical, Emergency Contact Information, and the Consent Form, which includes Concussion Awareness.
2. Physical: An athletic physical completed by your physician. Each physical is accepted for thirteen months from the evaluation date performed by a physician, physician assistant or nurse practitioner. Please upload the student-athlete's physical document under the appropriate section. If unable to upload electronically, please mail to Ted Quigley or Rob Riebe.
3. Rhode Island Interscholastic Assumption of Risk Form: All students in all grades must completely fill out this form and have it notarized. This form will only need to be completed once in each athletes high school athletic career.
4. Handbook for Student Athletes: Each student and parent must read and review the handbook, and agree to all policies and procedures set by La Salle Academy.
Students registering for Athletics must have a current physical examination on file in the Athletic Office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the Athletic Department no later than 3 school days prior to the first day of practice/tryouts.
